Please also note that any application to the scheme for the 2010 / 2011 year must be completed using this new form.
Creative NZ have produced this format to be consistent nationally.
Arts Council Nelson, (as Nelson City CCS Administrators), are unable to modify the form and apologise that this now spreads across eight pages which you are obliged to print.
